Job Title Behavioral Health Consultant / Licensed Clinical Social Worker

Summary: Assists patients and their families with personal and environmental difficulties through assessment and individualized intervention.

Essential Duties and Responsibilities: include the following. Other duties may be assigned.

Works in close collaboration with physicians and other health care personnel in patient evaluation and treatment to further their understanding of significant social and emotional factors underlying patient's health problem.

Performs screening/evaluation of individuals for mental health and substance abuse disorders

Provides brief interventions for individuals with behavioral problems

Provides behavioral support to assist individuals in improving health status and managing chronic illnesses

The Behavioral Health Consultant both meets regularly with primary care team to plan care and discuss cases, and exchange appropriate information with team members in an informal “curbside” manner as part of the daily routine of the clinic

Assisting with Integration of behavioral health into Primary Care

Providing intervention that is specific and goal driven

Providing education using handouts, “teach back” strategy, and tailored for each specific issue

On-going feedback to provider that is clear, concise, and BRIEF, focus on referral question, description of action plan

Education and/or Experience:

Five years of college or university program certificate; or five years or more related experience and/or training; or equivalent combination of education and experience such as:

MSW degree from accredited college or university, Licensed Clinical Social Worker, Licensed Psychologist, Provisional Licensed Psychologists, Licensed Master Social Worker, Licensed Professional Counselor, Provisional Licensed Professional Counselor, Licensed Marital and Family Therapist, Provisional Licensed Martial and Family Therapist

Language Skills :

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

Mathematical Skills :

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

Reasoning Ability :

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

Computer Skills:

To perform this job successfully, an individual should have knowledge of Database software and Word Processing software.
